If you are using an HMO plan, you must have a referral from your primary care physician. A referral from a dentist cannot be used for HMO plans. Please visit our patient referrals page for the form.

Please contact our office before your visit to confirm your coverage and remember to bring your insurance card with you to your appointment. Our team can file your claim on your behalf and can provide you with a super bill for direct reimbursement from your carrier.
